communication via modbus

We have some flow computers that each one does have a some ports which are Rs232 and the other could be set to Modbus,.
I want to know how I can connect the flow computers to a computer and gathering/monitoring thier data /measured values through modbus.
I would greatly appreciate if one could advise me.
